Registered Nurse Supervisor/Consultant

Waianae

Alternative Care Services, Inc. is seeking a compassionate, reliable and detail oriented Registered Nurse to provide supervision and consultation services for individual participants residing in our Community Affordable Rental Program (CARP) in Waianae. Primary responsibilities include assessments, creation of Individual/Nursing Plan(s), training licensed and unlicensed staff to implement plans, conduct monthly observations to ensure individual and nursing care plans are implemented properly by staff. Train staff and families on administration of medications and treatment plans ordered by medical doctors, dietitian, and other medical professionals. Train and supervise staff on proper documentation of nursing care plans on an on-going basis. Prepares and submits written progress reports on a monthly, quarterly, and annual basis to the assigned Department of Health Case Manager, legal guardians and caregivers.

RESPONSIBILITIES:

Train Direct Support Workers (DSW), licensed and unlicensed staff to implement prescribed medications and treatment plan prescribed by medical doctor, dietitian, and other professional consultants;

Schedules medical appointments, transcribe doctors orders, and train staff to implement nursing/treatment plans;

Performs technical nursing procedures such as, but not limited to tube feeding, administration of medications via g-tube, application of external catheter, suctioning, use of fleet enema, range of motion exercises, fecal impaction, and minor first aid and wound care;

On an on-going basis, order medications and supplies for prescribed treatment to ensure continuous availability;

Receives and properly processes the physicians and other professional consultants verbal or written orders;

Ensures a safe, orderly and clean environment for residents and staff;

Reports and submits reportable Adverse Events according to mandated procedures;

On a monthly and on as needed basis, provide supervision to licensed and unlicensed caregivers to ensure proper implementation of Individual Care Plans;

Analyze compiled data from Flow Sheets, Case Notes, ABC Behavioral forms, and other record keeping documents to determine progress or regression of clients status;

Conduct and submit monthly, quarterly, and annual written progress reports to the Department of Health Case Manager and Legal Guardians.

Required Skills:

Word computer program for record keeping documentations;

Good time management and organization skills;

Good verbal and written skills;

Confidence to teach, train, and demonstrate nursing care plans;

Flexibility to changes depending on medical/nursing needs of clients;

Hawaii Drivers license or must have reliable transportation;

Minimum Qualifications:

Current Registered Nurse license;

Reliable, self- motivated, and good judgment;

Position requires standing, bending, lifting, sitting, pushing, and walking;

Ability to remain calm and implement appropriate actions during emergency situations.

Possess good written and verbal communication skills to report and document required progress reports;

2 Step TB clearance or negative chest x-ray;

Drug and criminal background clearance;

Clearance for Adult Protective Services and Child Abuse and Neglect Registry;

Physical exam by personal physician;

Meet qualifications to work in the United States;

Pass State of Hawaii and FBI fingerprint background check.

Position Benefits: Full time (40 hours per week)

Initial pay rate: $30.00 to $35.00 per hour (dependent on previous work experiences)

Pay rate increase contingent on satisfactory completion of 90 days Probationary Period

Vacation leave: 10 days per year after completion of 2080 hours per year

Sick leave: 10 days per year

Holidays: 13 days per year (after completion of probationary period)
