GENERAL DESCRIPTION
RYSE seeks a Youth Development Specialist (i.e. case manager) to facilitate assessments, develop and monitor the individual success plans ensuring that all goals and objectives are met, and provide follow-up services for youth who have experienced homelessness. Position will serve as an advocate and liaison for and on behalf of all clients.
JOB RESPONSIBILITIES:
Provide individual case management and build and sustain cooperative relationships with each client on
assigned caseload.
Assists clients with obtaining necessary credentials (i.e. birth certificate, social security card, certifications etc.)
to establish identity and meet program enrollment requirements.
Sustain appropriate referral sources to best assist youth towards independence.
Develop individual success plans for clients that identifies barrier and appropriate resources (i.e. housing, childcare, transportation, healthcare/mental health, etc.) to aid in successful placement.
Develop employment and vocational plans with goals and timelines that will help clients achieve their personal, academic, workforce and housing goals.
Refers clients to appropriate partner/agency for mental health, substance abuse, education, employment etc. to ensure success.
Evaluates, facilitates and monitors services and clients progress in relationship to established goals and objectives.
Provide one-on-one and group instruction (i.e. job readiness, customer service, life skills, self-esteem, problem-solving and pro-social workshops).
Models appropriate behavior for clients, motivates youth, and reinforces appropriate behaviors.
Serves as a liaison for clients and offers appropriate interventions when necessary provides information and support; assist clients in completing various forms and obtaining documents for successful re-integration into community.
Plans and implements special events (recruitment fairs, job fairs, recreational and cultural events.
Participates in various community partnerships, collaboratives, work-groups, consortiums and/or boards.
Work cooperative, collaboratively and efficiently with program partners
Assists with the creation of professional job search tools including resumes, cover letters, portfolios, etc.
Documents and maintains complete and accurate member records to properly track clients progress through program.
Complete timely input into data management information system
Performs other related tasks as assigned.
JOB REQUIREMENTS:
Associates Degree or Bachelors Degree in related industry or field (human services, counseling, education, etc.) OR
5+ years of experience working in case management environment with a particular focus on youth and young adults
