Job Requirements:

Must be a MSW or have a Baccalaureate degree in a field of Social Work. Social Workers need to be licensed for each state they will be providing services in. With the Baccalaureate degree in Social Work or like field with 1 year experience in a health care setting. Must be a licensed driver with an automobile that is insured in accordance with state and organizational requirements. Demonstrated ability to take direction and respond to the needs of the patient and their families in varied settings. Knowledge and commitment to hospice philosophy. Criminal history clearance.

Adherence to the Standard of Behavior and consistently demonstrate Family Core Values that affirm life in all aspects of job functions.

Conduct a Biological/Psychological/Social/Cultural assessment of needs for patient and family.

Conduct financial assessment for Patient/Family and assist with need.

Provide supportive counseling to Patient/Family as needed to help resolve distress.

Collaborate with Patient/Family and team members to create comprehensive plan of care that is patient driven.

Provide Bereavement Services that are theoretically, culturally and developmentally appropriate to Patient/Family, staff, support groups, and community members.

Document patient charts in accordance with DAROP charting.

Our amazing Medical Social Services team provides many resources to our patients and their family members to help them resolve issues and concerns at end of life. Heart 'n Home hires Social Workers and Bereavement Counselors who want to play a role in helping the entire family and patient to cope with death and potentially reconnecting family members, if needed. Our team also takes care of the patient's family with grief and loss support for up to 13 months after the loss of their loved one. Without this support, many families would not heal appropriately. We feel like the Social Work and grief and loss support provided to our patients and families is vital to the success of our mission and vision.

Compensation:

$45,760 - $54,080/year Depending on Experience
