The Family Medicine Residency of Idaho is recruiting for a Community Health Worker to join our team.

The Community Health Worker/Certified Enrollment Counselor (CHW/CEC) is an important team member supporting the Outreach & Enrollment department, which is essential for achieving the dual mission of FMRI. The CHW/CEC shall be accountable for 1) connecting patients and their families with local healthcare services and other community services resources, 2) assisting patients and their families in the adoption of healthy behaviors, 3) providing community outreach services, social support and informal counseling and advocacy for community health needs, and 4) helping to provide services such as glucose test, and blood pressure screening. Community outreach, such as home visits and helping with health screenings at community fairs will be required.

The CHW/CEC works closely with receptionists, providers, nurses, medical assistants, counselors, dietician and referral coordinators as well as team members from the pharmacy and business office departments.

This is a full-time position based in Boise. Due to the nature of CHW/CEC work, day to day work hours may vary based on patient needs and the CHW/CEC participation in community events. Given the aforementioned, this position's usual schedule is Monday through Friday: 8:00 AM to 5:00pm with a one hour lunch break.

Qualifications:

Education: High School Diploma, Certified or Registered Medical Assistant preferred.

Experience: 3 or more years of experience in the medical field required to include Medical Reception, Social Services and Nursing.

Licensure / Certification: CMA / RMA preferred

Preferred Knowledge, Skills and Abilities

Bilingual ability and/ or qualified medical interpreters preferred.

Must have strong customer service and interpersonal skills,

The ability to collaborate with other health care providers in the provision of patient care

Ability to multitask in a busy environment.

Basic computer competency skills

Personal time management skills

Strong detail orientation
