We are seeking a part/full time person with great organizational and self-motivated skills to enhance the life of adults with developmental disabilities. The successful applicant will have excellent people skills, be able to handle demanding situations and celebrate accomplishments. This is a contract position with extremely flexible hours.
Responsibilities:
Linking to services and opportunities in the community that will increase independence and address individuality
Coordinating services with Medicaid, Social Security, Vocational Rehabilitation and many other providers to assure basic needs are met
Monitor services to assure goal accomplishments, safety and well-being
See adults in their homes and community
Area of services are flexible to include Idaho Falls, Rigby and Rexburg areas
Service Coordinator skills:
Bachelors degree in a human services field
Minimum of 1 year experience with the developmental disability population
Working knowledge of Word and Excel programs
Excellent follow through skills
Ability to problem solve and find resources and solutions for various needs
Willingness to research and find community resources & activities that would promote and assist with reaching full potential
