We Are HiringHome Health RN Coordinator Full-Time: Monday-Friday 8am -5pm Looking for those Nurses who love to care for others, are eager to help and grow. Those that not only take pride in their job but love what they do to join out Home Health team. This role wll allow you to utilize nursing and case management skills. Work independently with a team to support and coordinate with. Work with those vulnerable patients but also their families to assure they receive great care from our Nurses. What You Will DoProvide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.Implement and monitor patient care plans.

Monitor, record, and communicate patient condition as appropriate.

Serve as a primary coordinator of all disciplines for well-coordinated patient care.

Note and carry out physician and nursing orders.

Assess and coordinatespatient's discharge planning needs with members of the healthcare team.

What You Will NeedLicensure / Certification / Registration:BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date orjob transfer date required.

Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or jobtransfer date required.

Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) preferred.

Education:Diploma from an accredited school/college of nursing required.

Additional PreferencesPrevious home care experience preferred.
