  • Partner with Community Physician to ensure the community meets utilization goals, operational and clinical outcomes
  • Coach, mentor and counsel APCs and SWs to meet performance levels, and foster growth of “Care Community”
  • Previous experience in discussing advanced directive, goals of care, and end of life conversation to patients/families, and care givers.
  • Root causes analysis of suboptimal patient and community resource(s) outcomes to develop care plans to achieve optimal patient outcomes
  • Mentors APCs in developing medical, psychosocial, emotional, spiritual care plans and urgent treatments
  • Collaborates with the Community Physician, Clinical Liaison and Paramedics to meet the needs of patients transitioning from the hospital, facilities or patients in crisis
  • Orders, interprets, and analyzes diagnostic tests and determine the need for follow-up appointments and further treatment options
  • Prescribes and manage medications and treatments as appropriate, including but not limited to psychopharmacological agents
  • Updates patient records and check records for accuracy at each patient appointment
  • Initiates and carries out performance improvement plans, orientation and ongoing evaluation of APCs and SWs
  • Able to deliver and train on end of life and palliative care
  • Works closely with talent acquisition to hire high quality APCs and SWs
  • Teaches the clinical and nonclinical team members in formal and informal settings
  • Monitor and participate in the engagement of patients when attributed to their community
  • Accountable for the overall delivery, safety, quality and consistency of patient care
  • Ensure WellBe’s mission, vision, and values is embedded within market culture
  • Other tasks needed to accomplish team’s objectives/goals

QUALIFICATIONS

Educational/Experience Requirements:

  • Minimum Master’s Degree in Nursing
  • Valid nurse practitioner license in state of operations & current board certification in your field
  • 5+ years of progressive care required
  • 5+ years in a leadership role/experience required
  • BLS and ACLS certified and an NPI# as an NP
  • Have a valid driver’s license, car insurance, have access to a car and willingness to drive to patient homes/patient location, as well as an active auto insurance policy

Required Skills and Abilities:

  • Role model in courageous teamwork, integrity, ethics and productivity
  • Expert in diagnosis and treatment of chronic and urgent medical/psychological conditions of older adults
  • Very strong leadership qualities and professional demeanor
  • Experience in end-of-life, geriatrics, internal medicine, palliative care
  • Expert in palliative care, and end of life communication
  • Excellent verbal, written, presentation, and interpersonal communication skills
  • Confident speaker to internal and external groups
  • Strong computer skills, including Word, Excel, and Powerpoint
  • Home care experience preferred
  • Superior skills and experience in managing, training and ongoing mentorship of APCs
  • Ability to connect immediately with patients and families to contribute to difficult cases
  • Excellent time management and multi-tasking skills

Supervisory Responsibility: This position will have direct management authority .

Travel requirements

: Travel may be required up to 100% locally.

Work Conditions: Ability to lift up to 20lbs. Moving lifting or transferring of patients may involve lifting of up to 50lbs as well as assist with weights of more than 100lbs. Ability to stand for extended periods.

Ability to drive to patient locations (ie. home, hospital, SNF, etc). Fine motor skills/Visual acuity

Our Lead APCs are critical to the success of the Clinical Community that provides compassionate care to the frailest elderly who have chronic and acute illnesses or injuries living in their homes. The Lead APC will have direct responsibility of APCs and SWs outcomes within their community. The Lead APC will partner with the designated community physician to drive clinical outcomes, team development, and daily needs of the staff withing their community of patients. The Lead needed the Lead APC will deliver primary, urgent and acute medical care WellBe patients within the community. . Leaders in the clinical team will contribute to and facilitate onboarding of new APCs and SWs, IDT Think Tanks, structured ongoing education. You will also implement novel ways to improve clinical outcomes for the field. Lead APC's will facilitate an inclusive team culture consisting of physicians, social workers, paramedics, and care team coordinators to assist in delivering concierge care to every patient.
