Job Description
PAN Nurse: officially we title this job a PAN Coordinator, but in other centers it may be called a Post Acute Network (PAN) Coordinator or PAN Nurse (RN).This person will oversee all aspects of our new Post-Acute (PAN) programs. This will involve developing and managing quality measures, coordinating communication with our PAN partners, and ensuring that clinical needs are being met. You will also be supporting the DON in budgetary, survey & reporting needs. Your days will be rewarding and busy.

Qualifications
What we need from you:
- prior experience asPAN Nurse in a post-acute rehab unit/facility
- Registered Nurse (RN) in IL
- Strong technology skills (you are “Tech Savvy”), specifically with PCC
