This individual assists to develop and encourage a teamwork approach within the facility as it pertains to clinical issues. Under the supervision of a Registered Nurse or Licensed Practical Nurse, provides quality care and safe supportive services, which assist the resident in the achievement of physical and emotional support and comfort.Provides and maintains personal care services and proper hygiene to the resident and his/her environment. Care includes:

Assuring that residents are properly and comfortably clothed at all times.

Providing tub/showers with shampoos as directed.

Assist with shaving oral hygiene, finger nail trimming and toileting

Make beds and maintaining orderly neat rooms for the residents

Encourages resident independence in hygiene, dressing and ambulation.

Serves meals and nourishments, assists in walk and dine programs, assist in meal set-ups and feeding.

Reports intake and lack of appetite to charge nurse.

Assist and or performs the following procedures:

Admissions and discharges

Ambulation and transfers

Vital signs

Measuring I & O and specimen collection

Post Mortem Care

Routine Turning Demonstrates proper infection control techniques when administering care to prevent spread of infection and diseases.

Documents required information on appropriate forms (i.e., C N A Tracker, Skin Checks).

Demonstrates knowledge of resident safety and abuse prevention programs.

Participates in restorative programs i.e. Bowel and Bladder, ADL’s, hydration efforts.

Participates in resident activities.

Recognizes and reports any changes in resident condition (physical, mental, emotional, social) to the charge nurse.

Fosters self-respect and esteem by recognizing, accepting and respecting the residents entrusted to his/her care.

Cleans used equipment and returns to proper location (wash basins, bed pans, urinals, etc.).

Answers call light promptly.

Participates as a team player and assists other staff as requested.

Assist families and visitors when appropriate. Attends in-service programs and staff meetings.

Contributes to Care Plan conferences and Stand Up Meetings.

The CNA is responsible for implementing the facility’s Culture Contract through their interaction and implementation of their role with residents, families and staff.

Assists other team members when necessary.

All other duties as assigned.

Certified by the State of Illinois as a Nurse Assistant (Federal and State Law Requirements).

On the Illinois State Nurse Aide Registry with proper credentials and a clear record.

Current CPR Certification.

Ability to read, write, speak and understand English.

Long Term Care experience preferred.

Must be at least 18 years of age.

Ability to bend, stoop, squat, crawl, climb-height, reach above shoulder level, crouch, kneel, balance, push, and pull.

Because residents sometimes need immediate help to prevent a fall, and because such incidents are unpredictable, each CNA must at all times be capable of meeting the required lifting requirements up to 100 pounds without assistance or accommodation.
