Job Description
Plans and provides patient care to achieve optimum patient outcomes. Employs a collaborative process for assessing, planning, implementing, coordinating, monitoring, and evaluating the options and services required to ensure that patients receive the appropriate level of care and services to meet their ongoing healthcare needs.
Qualifications
EDUCATION: Associates Degree required; BSN preferred EXPERIENCE: Three years nursing experience or one year nursing experience and two years behavioral health experience required. Behavioral Health nursing experience strongly preferred. LICENSE OR CERTIFICATE REQUIRED: Licensed as a Registered Nurse in the State of Illinois; BLS required within 60 days of hire. ANNUAL REQUIREMENTS: Complete mandatory training and health system requirements
