Job Description

The

Diabetes Educator Dietitian reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

Responsibilities:

  • Provides diabetes self-management training (DSMT) to patients with diabetes and their families.
  • Conducts patient assessments and provides diabetes education as needed for inpatients, and provides education to individuals or groups at the Diabetes Education Center and in the community.
  • Coordinates and participates in health events to promote diabetes awareness and prevention.
  • Follows the standards set by the American Diabetes Association in practice and to maintain ADA recognition.
  • Provides assessment, evaluation and follow-up of patients with diabetes in the outpatient Diabetes Education Center.
  • Completes documentation of education, goals and follow-up of patients receiving individual or group diabetes education.
  • Communicates to patients’ physicians about education plan and needs.
  • Serves on Diabetes Advisory Board and follows standards set by the American Diabetes Association to maintain ADA recognition.
  • Assess patients with diabetes and support people for educational needs, abilities, learning preferences, and readiness to learn.
  • Identifies educational needs and counsels patients in diabetes self-management and medical nutritional therapy.
  • Provides information and instructions at a level of understanding appropriate to the comprehension level of the patient and/or significant other.
  • Enlists the assistance of and supports nursing/other departmental staff to implement diabetes education.
  • Maintains appropriate documentation in patient records.
  • Reviews, updates, and makes recommendations concerning diabetes related patient education materials as needed.
  • Encourages continued education for patients and appropriate management of their needs by referral to outpatient education when appropriate.
  • Participates in community awareness activities related to the prevention, detection, and treatment of diabetes.
  • Coordinates and participates in health events.
  • Coordinates with Marketing to promote diabetes education and awareness programs to be offered to the community.
  • Serves as a contact and resource person for community members inquiring about diabetes education or treatment needs.
  • Identifies educational deficits of health care professionals and coordinates with Nurse Directors and Clinical Development Coordinators to provide in-service education programs related to diabetes education.
  • Assists with orientation of new staff as needed.
  • Seeks educational and learning experiences to maintain knowledge and current skills in theory and concepts of diabetes education.
  • Participates with appropriate departments and committees in the development and implementation of new and/or revised policies and procedures related to Diabetes Education.
  • Participates in performance improvement projects related to the diabetes program at the Diabetes Education Center for American Diabetes Association (ADA) Recognition and within the hospital if needed.

Qualifications

Required:

  • Associate’s degree
  • Two to three years in diabetes education
  • Registered Dietitian in the State of Illinois

Preferred:

  • Bachelor’s degree in Nutrition or Dietetics
