Apply today!This is a Telephonic Case Manager position and is telecommute / work from home from any location in the USA. Does require one late day per month/ no weekends.You’ll enjoy the flexibility to telecommute from anywhere within the U.S. as you take on some tough challenges.Primary Responsibilities:
Make outbound calls to assess members' current health status
Identify gaps or barriers in treatment plans
Provide patient education to assist with self-management
Make referrals to outside sources
Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
Support members with condition education, medication reviews and connections to resources
This is high volume, customer service environment. You’ll need to be efficient, productive and thorough dealing with our members over the phone. Strong computer and software navigation skills are critical. You should also be strongly patient-focused and adaptable to changes.Required Qualifications:
Current, unrestricted RN license in the state of residence
Multiple state licensure (in addition to Compact License if applicable) or ability to obtain multiple state nursing licenses (application and fee paid for by UHG)
3+ years of nursing experience in a fertility clinic where working with fertility patients was the main focus of your position (OBGYN experience doesn’t qualify)
Strong clinical knowledge in the field of infertility with the ability to provide education to member who are seeking infertility treatment
Intermediate proficiency, including the ability to type and talk at the same time and to navigate within Microsoft Windows
Access to high speed internet from home
Dedicated work space
Ability to work one late shift a month
Preferred Qualifications:
BSN
A background in managed care
Knowledge of 3rd party reproduction
Willingness to obtain CCM within 30 months of employment
Case management experience
Certified Case Manager (CCM)
Experience or exposure to discharge planning
Experience in utilization review, concurrent review or risk management
Previous experience in a telephonic role
