We Are HiringAccountable Health Communities (AHC)Position description: The Community Health Worker/ Screener and Navigator position within the Accountable Health Communities (AHC) model at AMITA Health is primarily responsible for conducting on-premise screenings for health related social needs (HRSN) among the Medicare/Medicaid patients who present to the Emergency Departmant (ED) for care. In addition, the Screener/Navigator will assist from time to time with providing community services navigation to these patients.Job Requirements:With training, demonstrate comprehensive understanding of the AHC Program requirementsWith training, demonstrate comprehensive understanding of the standard operating procedures for screening in the ED setting as well as community service navigationFunction effeciently and effectively in the Emergency Department setting, as well as other clinical delivery settings as directed, with comfort and professionalismConduct screening in a patient-centered, compassionate mannerManage all administration of the screening including but not limited to ensuring all supplies are available and screenings are transmitted timely and accruately to the AHC Hub Office and supporting full compliance with the requirements of the Accountable Health Communities ModelExhibit excellent communication, documentation, and customer service skillsWith training, command a working knowledge of the AHC CMS Data System and other systems required to administer screening in the AHC model (Aunt Bertha)Additional duties as assignedObjectives:Contribute to the AHC Program milestone goals by completing AHC screenings (and/or navigation) for all eligible patients who present in the ED during shift.Actively participate and contribute as an AHC Team member to innovation, process improvement, and goal achievementDemonstrate AMITA Health mission, values, and behaviors at all times Other:Position may require shifts at different ED locationsFull time (40 hours) position and working hours can be flexible and include weekendsWhat You Will DoSupport health plan members by meeting non-clinical care management needs.Provide members with wellness coaching in person, virtually and by phone

Assist members with applications for public aid, payer specific programs, accessing medication, disability benefits, and other non-clinical issues as needed.

Locate and facilitate access to community resources for members.

Provide documentation support related to care management

What You Will NeedLicensure / Certification / RegistrationBLS Provider credentialed from the American Heart Association (AHA) preferred.

Education:High school diploma or GED required.

Additional PreferencesHighly Preferred: Bilingual in Spanish and English

Must be able to train in Elk Grove Village

Prior experience in healthcare
