Job Description
Collaborates and participates in the coordination of screening, assessment, and discharge planning activities in high risk patients and their families with complex psychosocial and economic needs. Utilizes expertise and problem solving skills to achieve optimal clinical outcomes within effective resources and timeframes. Promotes a seamless and safe discharge throughout the continuum of care.
Qualifications
Requirements: Minimum: Minimum of one full year experience and/or internship in a medical social work role in an acute hospital setting or agency. Must have prior experience with maternal-child populations in order to work in these areas of specialty; Preferred: Two ye Education: Preferred: LCSW preferred. Licensure: General: Must be licensed and maintain a current license or be licensing eligible. If licensing eligible must become licensed within 6 months of hire; Preferred: LIC LCSW
