Overview The Admission Team Leader will assess patients for hospice appropriateness on an individual basis once patient choice has been elicited from appropriate hospital staff and/or referral is made to Hospice from other referral sources. Explains Memorial Hospice program services to patient/family and functions as a liaison between referral source and hospice department. Proactively educates physician and family, and acts as a resource for Memorial health affiliates, referral hospitals and nursing home staff regarding Hospice services, Medicare coverage, regulatory limitations for admission, etc. Good understanding of Medicare guidelines and documentation requirements for reimbursement.

Qualifications Education:
  • Graduate from accredited school of nursing.
  • BSN preferred.
Licensure/Certification/Registry:
  • IL Licensed Registered Nurse required.
  • CPR certification required.
  • Must possess a valid IL driver license and must be deemed an acceptable driver in accordance with the MHS Fleet Safety Policy (five year MVR will be required).
Experience:
  • Minimum two years nursing experience required. Two years of experience in home health/hospice services required.

Responsibilities
  1. Establishes a close team relationship with referral sources, hospital case managers and discharge planning, as well as discharge planners of other facilities and/or hospital departments.
  2. Explains program philosophy, care issues and practices, and regulatory restrictions to referring hospitals, physicians and others in order for optimal care to be achieved. Serves as a resource to Memorial Health personnel, other agencies and hospital personnel and the community.
  3. Upon referral, contacts and interviews patients and families to assess and evaluate patient's social, financial and environmental situations. Explains hospice philosophy, benefit coverage and eligibility for hospice services. Evaluates patients to determine hospice eligibility. Triages referrals based on needs.
  4. Upon referral, works with the case manager to develop a discharge plan for the hospitalized patient based on patient/family needs. Works with patient, family, physicians and staff to promote safe, appropriate and timely hospital discharge. Coordinates physician/family referrals and start of care. Works with the ordering physician to develop an appropriate initial plan of care. Works with Insurance Liaison to determine coverage issues. Informs family of coverage for hospice services.
  5. Communicates/coordinates referrals with Clinical Manager
  6. Participates in Hospice team functions and quality safety improvement projects as requested.
  7. Maintains statistics as requested.
  8. Follows agency policies and procedures.
  9. Assists the Quality Coordinator in QAPI data collection, trending, analysis, and reporting.
  10. Demonstrates extensive knowledge of hospice regulatory guidelines and ensures compliance with these.
  11. Triage calls from outside the office and implements necessary communication to staff, physician offices, and patients regarding information.
  12. Coordinates day to day activities of the clinical team for efficient operations and appropriate patient care of the department, including scheduling based on assessment of patient needs.
  13. Participates in MHSvcs performance improvement activities through chart audits, data collection, and focus groups.
  14. Requires in-depth knowledge of Medicare requirements and Conditions of participation to review documentation and 485.
  15. Responsible for receiving and documenting resolution of customer concerns. Communicates unresolved issues to supervisors for further follow up.
  16. Responsible for monitoring and gathering Quality Improvement data and logging into Excel Spreadsheets.
  17. Performs other related work as required or requested.
