Social Work Case Manager-PRN- Los Angeles, CA.Location: United StatesNewRequisition #: PS34709The Field Social Worker is responsible for

providing supportive and medical social services to patients and their families

in the home.Ideal candidates will have experience identifying and coordinating

the needs of chronically ill patients and families as well as supporting

clinicians to help them operate as efficiently as possible.Primary duties may include, but are not limited to:

Provide in home and telephonic psychosocial assessment andsupportive services to patients and families as part of an interdisciplinaryteam.

Care coordination with the patient, family, caregivers, and otherproviders includes: .

Supporting hospice consultationand transition.

Goals of care discussions andeducation on treatment plan recommendations at end of life.

Social support andtransportation needs.

Financial counseling.

Identifying non­clinicalcommunity resources.

Mitigating family dynamics andpatient compliance issues.

Assess, identify, and closeclinical and non­clinical gaps in patient care.

Appropriate triage andescalation of routine, urgent and emergent patient calls.

Document social workassessments, care plans, and interventions

Collaboration with field-basedNPs/PAs and physicians to develop and implement effective treatment plansand interventions for complex patient care issue

Identifying community resourcesand assisting patients and families in accessing those resources

Referral to chaplain resourceswhen appropriate

Provide grief support andreassurance to patients and family members

Participation in weeklyinterdisciplinary team meetings

Perform miscellaneousjob-related duties as assigned

Knowledge/skills/abilities:Regular attendance is an essential function of the jobPrior healthcare experience serving chronically ill patients living with limited resources, and/or mental illness preferredHealth plan, home health, or hospice experience a plusTelephonic case management or care coordination experience preferredStrong customer service orientationPatient, compassionate, and professional demeanor requiredExceptional organizational skills and ability to multi­taskSelf-starter with an entrepreneurial spirit and demonstrated problem solving skillsStrong writing, Word, Excel, and database management skillsExtremely strong work ethicExperience working in a fast-growing, rapidly changing environmentMust have reliable automobile, valid driver’s license and minimum state required liability auto insurance; Social Workers travel in personal vehicle with reimbursement for mileage.Education and experience requirementsMaster’s degree in social work requiredLCSW preferredMinimum of 3 years social work experience required,

preferably in a centralized care management team environmentRegulatory RequirementsRequires adherence to all internal policies, processes,

and procedures ensuring compliance with customer expectations and all

applicable laws and regulations for privacy, security, and social

responsibility, especially HIPAA and HITRUST.Physical RequirementsThe physical demands described here are representative
