The Home Infusion Pharmacy Technician assists the Pharmacist by filling physician orders for intravenous (IV) and subcutaneous (SC) medications, mixing IV/SC admixture solutions, coordinating care, preparing labels and other critical documents, and performing other related duties. Responsibilities: Prepares sterile intravenous and subcutaneous preparations in compliance with USP<797> guidelines using aseptic technique. Manages and maintains the integrity of the cleanroom environment per policy and best practices. Performs basic pharmaceutical calculations in order to confirm accurate sterile compounds. Fulfills patient orders, which include drugs and supplies, per delivery ticket. Arranges timely delivery to patient’s home while working with third party courier services. Conducts quality control activities centered on cold-chain delivery logistics. Assists in inventory procurement, storage, and management along with verifying the safety and integrity of products received. Organizes medical records such as delivery tickets and compounding records so as to comply with regulatory requirements. Identifies and reports medication-related incidences and equipment failures. Works within various software programs that support the operations of the department. Collaborates with Pharmacists to achieve positive patient care outcomes. Provides high-quality customer service to patients, vendors, and members of the healthcare team. Supports the quality improvement initiatives of the team which may include programs aimed at medication and supply stewardship. Participates in on-call responsibilities, as scheduled. Completes annual and ongoing professional competencies, as required. Other duties, as required.
