The Intake Evaluator II reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.Conduct non-emergency evaluations and admissions for all levels of care for mental health and substance abuse programs. Provide direct clinical intake services and consultation and have excellent communication with referral sources, internal and external customers.Description: Answers inquiry calls in a prompt and courteous manner, assisting callers, screening potential patients and setting up initial evaluation appointments.Conducts initial assessments for all levels of care for patients 12+ with addiction and mental health needs.Establishes rapport with patients and their families, assesses patient needs, establishes preliminary diagnosis and treatment plan recommendations.Completes written assessment in a timely, thorough fashion, completes required consents/releases with patients and communicates outcome of evaluation with referral sources with patient consent.Provides a system of smooth transition for all patients to the appropriate services within BHS or the community, working effectively with clinical staff, utilization review staff and business office to remove barriers to efficient provision of care.Works effectively with managers of all BHS programs to ensure patients are appropriately served and uses admission criteria appropriately.Communicates well with physicians, seeking their input for patient placement and notifying them of patient status as needed.Demonstrates initiative to problem solve in the moment to address patient issues as they arise.Works effectively with all referral sources, physicians, therapists, and contact persons to ensure the prompt and courteous assessment and transition to BHS programs or outside referrals as appropriate.Works cooperatively with BHS Business Development to foster current referring agent relationships.Knowledge of and proficiency with EMR/basic computer skillsRepresents BHS programs and access procedures to providers/referral sources within the community as needed.
