Job Description

The household coordinator is responsible for coordinating the services of the individuals served and assisting the Program Director with ensuring the individuals staffing needs are met. This position also has the responsibility to coordinate services including, but not limited to: individual finances, recreation, program services, family involvement, education/vocational programs, health care/nutrition, and household management. The household coordinator is responsible to assist the supervisor in maintaining consistency in programming, person served care, and communication with staff and interdisciplinary team members. The household coordinator is also responsible to carry out the responsibilities of a direct support professional during all shifts.

RESPONSIBILITIES AND DUTIES

  1. Ensure all individuals, their family members, community members and co-workers are treated with dignity and respect

  2. Assure individual, family, or community concerns are responded to promptly by communicating such issue to Dungarvin supervisors.

  3. Assist individuals to develop basic living skills including but not limited to social, domestic, and hygiene through instruction and encouragement.

  4. Assist in the development and implementation of short and long term goals for the individuals under the direction of the Program Director.

  5. Ensure the assigned group home has appropriate staffing levels in accordance with regulations or plans of care. Provide employees with routine schedules in consultation with the program director.

  6. Secure adequate staffing levels when responding to unexpected employee absences or changes in the individuals needs in consultation with the program director.

  7. Report concerns regarding employee performance to program director.

  8. Mentor, train, and orient new and current employees to any of the individuals served care and risk plans and procedures.

  9. Document the individuals’ progress of his/her short and long term goals as well as his/her daily activities using Therap or other documentation systems.

  10. Develop and maintain a positive and effective on-going relationship with individuals, families, staff, administration, other service providers, and case managers.

  11. Perform or assist individuals with personal hygiene as needed (e.g., bathing, brushing teeth, shaving, nail clipping, dressing, bodily functions, and individual appearance).

  12. Assist with personal care services that are assigned by a health professional which may include observing, reporting and documenting of changes in the health status of the person or in their bodily functions. Including but not limited to reading and recording a persons temperature, pulse or respirations or other needed personal care services.

  13. Perform or assist individuals with basic domestic duties as needed including but not limited to cooking, household cleaning, laundry, and shopping.

  14. Coordinate and assist with the maintenance of the individual’s schedule (e.g., doctor appointments, interdisciplinary team mtgs., and job schedule).

  15. Coordinate and assist with the individual’s participation in leisure and recreational activities.

  16. Maintain records and complete paperwork as required by Dungarvin including but not limited to Kronos time recording, mileage, petty cash, medication documentation, incident reports and goal data.

  17. Adhere to individual’s behavior and health management plans including but not limited to administration of medication, use of behavior modification techniques, and dietary restrictions.

  18. Maintain a safe environment for the individual; prevent harm to individual, self, and others.

  19. Report any and all incidents or safety concerns to the Program Director or emergency on-call supervisor in accordance with policy and procedure.

  20. Transport individuals to and from work, scheduled appointments, recreational activities, or other activities in a safe and timely manner.

  21. Manage individual’s and agency's funds in accordance with Dungarvin policies and procedures, and report any suspected or actual misuse of individual or agency funds or property.

  22. Serve as a positive role model to individual(s).

  23. Complete all required trainings prior to their expiration per Dungarvin conditions of employment.

  24. Keep up to date with current Dungarvin and regulatory licensure standard policies and procedures.

  25. Other duties as assigned.

Qualifications

Minimum 18 years of age. High school diploma/GED certificate is required Minimum of 2 years education, training, and/or experience in a related area is preferred. Requirements may be specific to individuals’ needs such as behavior management skills/experience and experience with high medical needs. Must be responsible, reliable, and able to carry out job functions independently with minimal direct supervision. Also must possess all of the following: ability to communicate in an effective manner; ability to make sound judgments when given guidance and priorities; ability to work as a team member to maintain consistency and quality in programming; ability to exchange support, constructive criticism, and ideas with other staff members.
