This candidate will be working closely with a lead physician seeing follow-up patients in the clinic and sleep lab patients. The schedule is Mon-Fri 8-4:30pm with shared evening call and weekend rotation. On-the-job training will be available, but sleep medicine experience is preferred. Previous Pulmonary or NICU experience is also preferred. We are not accepting new grads at this time.Responsibilities:Care Coordination: starting from referral process, to office visit, to orchestrating collaborative work with other subspecialties

Responsible for taking patient history, performing physical exam, making assessment/diagnosis, and formulating a care plan

Diagnostic testing, blood work, prescribing medication, and other therapeutic interventions (cognitive behavioral therapy, interpretation of PAP downloads, etc.)

What You Will NeedLicensure/Certification/Registration:Required Credential(s): BLS Provider credentialed from the American Heart Association (AHA) obtained within 1 Month (30 days) of hire date or job transfer date. Nurse Practitioner specializing in Prescriptive Authority credentialed from the Indiana Board of Nursing obtained within 6 Months (180 days) of hire date or job transfer date.One or more of the following: Registered Nurse credentialed from the American Nurses Credentialing Center (ANCC) obtained prior to hire date or job transfer date.
