Responsibilities and Duties:

The primary responsibility of the Targeted Case Manager (TCM) position is to advocate for a high quality of life for persons served and ensure appropriate coordination of all services provided to the persons served. This is achieved by ensuring compliance with various state/federal regulations and with the various CDDO regulations. The TCM's work is primarily performed in a variety of community-based settings.

Specific duties include:

Advocating for persons served with regard to lifestyle preferences, including means for gaining access to needed services and entitlements and seeking modification of those service systems when necessary to increase the accessibility to those systems;

Coordinating needed services and providing assistance to the person in being knowledgeable about the types and availability of community services and support options;

Maintaining compliance with state and CDDO regulations for each person served on assigned caseload. Completing BASIS assessments and related documents, risk assessments (as needed) and an Individual Justice Plan (as needed);

Creating the Quality of Life Plan (QLP) and encouraging participation of parents/guardians/families, other friends and staff in the QLP process. Facilitating the QLP meeting and preparing and disseminating the final product to appropriate team members and other Community Service Providers. Completing any addendums to the QLP document through out the QLP year to ensure the QLP remains an accurate reflection of the persons served preferences and life situation;

Arranging for and securing supports outlined in the QLP. Ensuring ongoing, proper implementation of QLP by monitoring activities and following up when the QLP and related supports are not effectively and adequately addressing the needs of persons served;

Assisting in the transition, including the planning of and arranging for services to follow persons served when they move or change services (including school to adult world).

Quality Assurance functions including communication with DCF Quality Assurance Coordinator, completion of CDDO quality assurance checks, and completion of assigned CLO internal quality assurance checks;

Working with the three KanCare Managed Care Organizations

Communicating appropriately with parents/guardians and other members of the community.

Additional responsibilities may be assigned as circumstances warrant

Qualifications:

The Targeted Case Management position requires:

Bachelor's degree in a human services or related field ;

at least six months of prior experience in a position of similar scope;

the ability to communicate through verbal and written mediums;

the ability to observe, ask questions, listen actively, and give and accept feedback effectively;

a high energy level, adaptability, assertiveness, patience and ability to work in a team environment;

knowledge of computers, specifically Excel and Word applications as well as Google email and Drive;

ability to work autonomously;

flexibility in assigned working hours;

time management and follow-up skills with ability to plan, organize and prioritize a large workload; and handle several tasks simultaneously; and

operation of a motor vehicle is required which includes proof of a valid drivers license, an ability to maintain automobile liability insurance as required by state law in Kansas and/or Missouri and an ability to qualify as a driver under CLO insurance guidelines.
