Vytalize specializes in providing expanded primary care services that improve the quality of care for patients and is leading the transition to value-based medicine.Visit www.vytalizehealth.com and www.thephysiciansallianceaco.com for more information.Job DescriptionWe are currently looking for a Registered Nurse to provide a variety of both clinical and administrative tasks and to support the work of physicians and other healthcare professionals in helping manage chronically-ill patients in an outpatient setting. The position involves helping the physician in conducting Annual Wellness Visits for patients, identifying preventive measures, creating care plans for chronic conditions, and conducting advance care planning for patients. The RN will be responsible for supervising the implementation of the care plans and behavioral coaching by other clinical staff. Each patient’s case should be looked at individually and reviewed in order to determine how it helps improve the patient’s quality of life and improve quality metrics for ACO. In creating a plan, patient ability and needs need to be considered. Within the plan, the nurse may include treatment methods, medication, nutrition, and exercises that need to be carried out by the patient. Among others, the RN is in charge of the following responsibilities:

Assess patient needs and develop a care plan.

Work with all providers to bring best quality care for patients

Fulfills patient care responsibilities as assigned by providers and supervising physician that might include collecting patient intake, scheduling, prepare and educate patients for their visit and/ procedures and relaying instructions to patients/families

Ensures that all patients’ records are updated and accurate in the EHR.

Salary: $60,000.00 to $85,000.00 /year Job Type: Full-time
