Description/Job SummaryPosition SummaryCrisis Evaluator- On- Call is a home-based position with community based components. The goal is the same: to provide immediate response to mental health, substance abuse or DD crisis via telephone and face-to-face interventions.Essential Duties and Functions
Assess client s mental/emotional status; obtain history from client, family and other collaterals; medical history if possible (med compliance, SI/HI, previous hospitalizations)
Develop and implement an immediate plan of action with client, natural supports and/or other organizations for managing the crisis; monitor intervention and reevaluate
Remain with client until crisis is stabilized or next level of care is implemented
Identify & refer appropriate resources within the social service community
Initiate supervision with either day or on call supervisor as directed by policies and procedures or as needed
Provide telephone and/or in person follow-up on crisis fact sheets, as well as MCS cases
Keep accurate case records including all required permission and referral forms
Attend monthly and/or quarterly staff meetings
Other duties as assigned
Qualifications
A Master s degree in Social Work or Counseling
Obtain licensure of LMSW or PLPC within 60 days of hire date
The ability to make ethical clinical decisions in demanding situations
The ability to interact professionally with clients, their families and other professionals
Organizational skills; ability to complete documentation timely and accurately
The ability to work as a team
Experience required: 0 – 3 yrs
