Full TimeDescriptionConducting navigation, linkage, adherence/retention support of clients in HIV/STI/Hepatitis C (HCV) services through linkage and referrals into other Prevention programming and agency wide services, with an emphasis of serving key populations. Work with linkage team and other agency departments and partners, including medical providers, to actively test, link, and navigate clients to services including medical care and essential support services.Ensure compliance with Center for Disease Control/Office of Public Health (OPH)/funder requirements and attainment of program and outreach deliverables.Have a working knowledge and ability to navigate electronic health record systems. Assist supervisor and managers with meeting program area requirements, including data management, analysis and reporting, staff and staffing needs, supplies and all quality assurance (QA) measures. Ensure all reporting and evaluation mechanisms are complied with.KEY DUTIES AND RESPONSIBILITIES
Provide client-centered navigation and support services to priority populations accessing prevention services, with an emphasis on HIV and HCV medical care
Conduct HIV/HCV/STI counseling and testing
Collaborate with other departments and organizations to coordinate services to ensure client needs are met.
Assist in data collection and data analysis, including electronic medical record follow up, to measure achievement of linkage deliverables for prevention grants
Conduct intensive support for adherence and retention to HIV care services, ART treatment, HCV treatment, STI treatment, and PrEP services.
- - Report on all linkage activities.
- Track all encounters with patients receiving linkage services.
- Organize and conduct linkage in the context of street outreach, community outreach, testing, and educational events, in community venues and social events in urban locations.
Assist with sexual health and harm reduction community education curriculum and marketing initiatives.
Maintain social media, geolocation settings, and help create original content.
Work with community organizations to build and foster linkage programs.
Help teach and present for organizations on sexual health, harm reduction, and HCV prevention.
Create and input survey data for reporting purposes and assist department leaders in ensuring all program deliverables are achieved, including ongoing monitoring, and pro-actively making adjustments as needed to ensure deliverables are on track.

- Assist with monitoring and evaluation. Complete all reporting requirements in timely and professional manner
- Work with other appropriate staff to implement OPH requirements on care and social service referrals and Pre-Exposure Prophylaxis (PrEP)
- Ensure compliance with Center for Disease Control and Office of Public Health funder requirements for project.
- Assist in creating reports, new articles, research papers, grant proposals, etc. that inform and promote the goals and results of the program and agency
- - Oversee day to day operations of program area, including ensuring staffing needs are met so program activities, such as HIV/HCV/STI testing and condom distribution events, have adequate staff to see clients in a timely manner and ensure all quality assurance measures are met, supplies are available, and all other logistical requirements are met.
- Work with other program areas and staff to meet program area needs and to communicate with other staff on program requirements.
- Other duties as assignedCandidate must possess below qualifications to be successful in role:
Bachelors degree AND 2 years experience in public health o relevant field (social services, research, client navigation, community health work and/or community engagement) field required OR High School degree/GED/HiSet AND 5 years experience in public health or relevant field (social services, research, client navigation, community health work and/or community engagement) required
2 years work experience with priority populations at high risk for Hepatitis C, including People Who Inject Drugs, required.
Bilingual Spanish preferred
