Northern Light Acadia Hospital Department: Addiction Medicine ClinicPosition is located: AcadiaWork Type: Full Time Hours Per Week: 40.00Work Schedule: 5:00 AM to 1:30 PMSummary:The Substance Abuse Clinical Intake Coordinator is an integral member of the interdisciplinary team and works under the supervision of the Clinical Supervisor for Adult Services/Substance Abuse. The Intake Coordinator is responsible for the daily preparation, implementation, and process/flow of the scheduled inductions to the Narcotic Treatment Program. The Intake Coordinator completes pre-induction paperwork, reviews all requirements with the inductee, and is the main contact throughout the process. He/she and is responsible for the whereabouts of the inductee and making sure the process moves efficiently. The Intake coordinator is a member of the scheduling team, making reminder of appointment calls, screening for verification, building/prepping induction folders, escorting clients through the induction process, complete EKGs as requested by medical staff, create/re-opening SMART charts for inductees, oversees completion of all requirements of the induction process prior to client dosing. The Intake Coordinator also gathers and enters TDS information into the State of Maine data base as well as assists counselors with various tasks including but not limited to: take home requests, updating releases, and entering data in SMART. The Intake Coordinator uses critical thinking skills to assess and advocate for patients in terms of unaddressed needs that are communicated to clinical/medical staff. He/she will also meet on a scheduled basis for individual supervision to identify progress toward goals and to ensure continuous competence of duties outlined below. Completes pre-induction forms with inductee: dual enrollment, provider information, consent to be photographed, TDS, physical history, PCP and PMP releases.
Obtains the inductee’s prescription history via use of the Prescription Monitoring Program.
Consistently informs necessary personnel of inductee’s arrival.
Escorts inductee to and from points of contact as part of the induction process.
Communicates with the FNP when the history and physical is due for completion.
Completes EKGs as requested by medical staff.
Constant oversight of inductee’s location and process flow.
Tracks wait time to be less than 20 minutes between providers needing to be seen.
Communicates with nursing staff for induction approval.
Opens SMART chart series for the inductee.
Provides tour of program, daily dosing expectations, and program rules.
Obtains urine drug screens.
Assigns inductee to a clinician and provides notification to clinician.
Assigns mandatory groups and initial appointment with counselor and provides the dates/times.
Data entry of all pertinent information into SMART once all documentation has been returned for providers involved in the induction process.
TDS form completion – transfer information to future discharge TDS.
All TDS information is entered into the State of Maine database system.
Prepares evaluation/induction folders.
Makes reminder calls to next day’s inductees.
Participates in daily scheduling meetings to review PIFs for induction criteria.
Types and emails induction schedule for the following week.
Updates during the week as changes take place.
Assists counselors with various tasks as appropriate for scope of practice.
Collaborates with other clinics, doctors or other provider offices to obtain verification as needed.
Demonstrates reflective/active listening, feedback, summarizing, reframing, empathy, support, problem solving, coaching, and psycho-education skills.
Maintains and abides by ethical standards set forth by the facility and Maine State Licensing Boards, if applicable.
Maintains requirements of licensure, if applicable.

Reviews all meeting minutes.
