Easter Seals Military & Veterans Services, in partnership with Veterans Count, responds rapidly, efficiently and effectively to the unmet needs of Service Members, Veterans, and their Families to ensure that they can thrive in their communities.

We are seeking a Community Integration Specialist/Care Coordinator in Portland to provide Rehabilitative service in the context of a supportive relationship according to the individual's needs and promote Recovery and Integration into the Community while assisting in stabilizing a current living situation. The person in this key role will also provide assessment, planning, counseling and service coordination to service members, veterans and their families.

Responsibilities: Perform the following assessments to assist in the development of the Individual Service plan for each eligible Veteran and/or person(s): Need for Change, Housing Needs Assessment, Community Support Services Assessment, Mental Health & Substance Abuse Disorder (AC-OK) screening, Adult Strength Needs Assessment (ANSA). Receive referrals of service members/families, respond promptly to new referrals and calls/requests from existing clients, complete comprehensive intake/assessment/care-planning process with service member and families and serve as "one stop" resource for coordinating services.

Qualifications: A Bachelor's degree with five (5) years related experience and strong clinical skills is required, Master's preferred. MHRT-C certification or the ability to obtain certification is required, and an understanding of military structure and culture is preferred. This position requires the ability to work as part of a team and a reliable vehicle and insurance which meets our coverage requirements.

Hours: This is a part-time (20 hours a week) in Portland. This position does require the ability to work some evenings and weekends.
