flexibilityWork hours: 8:30 am - 5:30 pm PST11:00 am - 8:00 pm EST10:30 am - 7:30 pm CSTThe NurseDisease Management I Position is responsible for participating in delivery of

patient education and disease management interventions and for performing

health coaching for members, across multiple lines, for health

improvement/management programs for chronic diseases.Primary duties may

include, but are not limited to:

Conducts behavioral or clinicalassessments to identify individual member knowledge, skills and behavioralneeds.

Identifies and/or coordinatesspecific health coaching plan needs to address objectives and goalsidentified during assessments.

Interfaces with provider andother health professionals to coordinate health coaching plan for themember.

Implements and/or coordinatescoaching and/or care plans by educating members regarding clinical needsand facilitating referrals to health professionals for behavioral healthneeds.

Uses motivational interviewingto facilitate health behavior change.

Monitors and evaluateseffectiveness of interventions and/or health coaching plans and modifiesas needed.

Directs members to facilities,community agencies and appropriate provider/network.

Refers memberto catastrophic case management.

Requires an AS/BS in nursing; 2years of condition specific clinical or home health/discharge planningexperience; or any combination of education and experience, which wouldprovide an equivalent background.

Current unrestricted RN licensein current state(s) required.

Acurrent unrestricted CA, NV, and/or WA RN license is required.

(You must have one of these currently to be considered, will have toobtain the others once employed).

Prior Case Management Experience Preferred

Manage care experience.

Previous work at homeexperience preferred.
