Social Worker

Build and maintain professional relationships with staff, physicians, residents, families, community resources, and third-party payers.Educate residents and families on Medicare, Medicaid, and financial assistance programs or refer them to the Finance Office.Complete admission forms, assessments, and required documentation.Develop and update individualized care plans, tracking progress and adjusting as needed.Complete the Social Service section of the MDS within regulatory time frames.Gather Advance Directive information, provide education, and assist in completion if applicable.Continuously assess residents’ mental health needs, provide education, and facilitate referrals to mental health services.
