Job Description

The Case Manager provides care coordination for a patient's hospital stay. Working with the healthcare team, the case manager monitors the LOS, the appropriate utilization of care, promotes quality, provides clinical review for the payers, and plans/implements the discharge. The Case Management Department upholds the current standards of case management practice and creates an environment that promotes the individual growth and development of its staff.

Responsibilities:

  • Manage a daily caseload which is specialty-based, in accordance with hospital/departmental policy and procedures and regulatory requirements. Optimize their work to accomplish the goals and strategic initiative identified for each fiscal year
  • This role is carried out in collaboration with the social workers, considering the care and psychosocial needs of both the patient and family. Foundation for this position will include; Integration of the best in evidence-based practice, professionalism and customer service
  • Prepare reports to inform the case managers and department of their outcomes and assists them in evaluating their practice and how to accomplish next steps

Qualifications

  • Bachelor's degree in Nursing is required
  • Current Massachusetts registered nursing license required
  • Ability to work with case manager technology and clinical leveling criteria
  • Able to learn and expand skill set as new protocols and projects are initiated
  • Possess the ability to transfer clinical knowledge/skill to the care coordination of their patients/families
  • Utilize critical thinking to case manager and implement a safe, appropriate discharge
  • Prior experience in case management or a related field is required
