Maxim Healthcare Group is seeking a Direct SupportPersonnel. The Direct Support Personnel assists with growth and development ofinterpersonal skills and Activities of Daily Living in the home or community toindividuals with MRDD or mental health challenges who live independently (withassistance) in their own residences (supportive living) or a communityresidential property. Essential Duties and Responsibilities: Provides training/assistance to assigned individuals withInterpersonal skills and Activities of Daily Living in accordance with theoutcomes and goals in the individual’s support plan and under the directsupervision of the Team Leader, Program Coordinator, or Program Director
Addresses all outcomes and goals as outlined on theindividual’s ISP and BSP are performed
No changes in care are initiated without direction from theTeam Leader
Any changes in status are reported to Team Leader, ProgramCoordinator, or Program Director
Documents care per company policies and procedures,evidenced by notes are completed, accurate and turned in on a timely basis, andin accordance with documentation policy guidelines
When unable to complete an assignment, for any reason,office leadership is informed immediately. Changes in assignment due toconsumer or family request must be verified. Maxim Healthcare Services must beaware of changes in assignment and reason for change. Confirms schedule weeklywith Team Leader/office leadership
Participates in Multi-disciplinary Team conferences andIn-services as requested, verified by sign-in sheets
Mandatory In-services are attended, verified by sign-insheet
Performs other duties as assigned/necessary
Minimum Requirements: High School diploma or its equivalent
Acceptable motor vehicle driving record
Reliable transportation
Current state driver’s license (or state ID if not drivingconsumers)
Current automobile insurance
Proof of 2 references or per state requirement
Current CPR card per state / client requirement
Current TB skin test per state / client requirement
Proof of six months MRDD experience preferred
First Aide certification per state / client requirement
Medication Administration Certification (in accordance withstate nurse practice act exemption) by end of introductory period per state /client requirement
Proof of mandated state training in accordance withrequirements of the Area Authority Provider Manual per state / clientrequirement
Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language may be required
Computer proficiency required
Must be at least 18 years of age
