Welcome Family Nurse Home Visitor

Collaborate with referral sourcesProvide outreach to potential participantsSchedule and conduct up to 90-minute visits at the family's home or by telehealthScreen in six core areas: clinical assessment, unmet health needs, maternal and infant nutrition, substance use, emotional health, and domestic violenceProvide brief intervention and education to all families, regardless of perceived risk or needAddress the caregiver's individual questions and concernsOffer referrals to services for caregivers with an identified need, including MA MIECHV-funded home visiting programs and other community resources that may best suit the needs of the familyEngage, assess, and present information in an unbiased, culturally appropriate manner, particularly related to sensitive topicsConduct a follow up contact with all caregivers 2-3 weeks post initial visitCollect data using the Welcome Family forms, complete required DPH and agency documentation and billingParticipate in the Welcome Family Learning Collaborative and DPH annual site visitComplete agency and state required training
