The Case Manager will be working to coordinate in home supports to individuals with disabilities. The position is full time with benefits. The hours are mainly 9-5 but may vary according to the needs of program participants. The Case Manager will be responsible for collaborating with individuals and their families to fulfill their vision of a fully meaningful and inclusive life in their local community.
Essential job functions include but not limited to:
Meet with individuals/families in their homes monthly or as needed.
Create service/goal plans and update regularly
Work in partnership with Human Resources to recruit Support Workers.
Monitor annual funding allocations to maintain accurate records of funds used.
Maintain regular correspondence with individuals/families and other interested parties.
Complete necessary referrals for services as needed.
Complete all forms and reports on time.
Qualifications: BA required with a minimum of three years of experience working with individuals with disabilities or related experience. Valid driver's license required.
