Description: Technology advancements are required to obtain the 2022 VBC affordability and quality targets of $168M (Medical Cost and Revenue), and to meet YOY incremental VBC benefits with the same contracted providers. For this business case, 33% of the $79M (FI Affordability target across all LOB) is being attributed at ($26.07M / year) to the 2022 Capital spend, while 67% of the benefit is dependent on the people and processes used to align and drive providers on practice transformation actions. Thus, 1/3 of the affordability is attributed equally across technology advancement, people, and process. The critical technology advancements include the ability to drive the expansion of the Spectrum (small provider) care management workflow; digitized clinical transformation workflow to drive predictive analytics; consolidated and curated data for provider feeds and data integrations; and a core, aligned and consolidated set of care management and population health capabilities for internal clinical teams, and external provider organizations. Business case takes half year benefits in 2022 due to build out and capability delivery timing. Currently we have a fragmented system across our care management and population health capabilities for client and our provider organizations. Current VBC reporting includes 40+ different reports, data marts and multiple teams supporting internal and external users. Since every team builds their own integrations, the result is misaligned data, use of tertiary data sources, multiple product roadmaps and multiple, sometimes competing, backlogs. We currently manage Population Health and Value-Based Care using 4 core tools including Practice Assist, Clinical Transformation Tool, Evolve and iPCA. Consolidating these tools and bringing the 40+ different reporting sources together will help: I. Create a streamlined user experience and aligned technology experience for internal and external users. II. Improve workflow and productivity for clinical transformation activities. III. Create accessible, timely, streamlined and enhanced performance reporting to improve effectiveness of our Care Management, Value Based Care and Population Health efforts. IV. Support timely incentive calculation and adjudication capabilities to include more providers in Value Based Care arrangements, specifically in Spectrum and HPP programs. V. Integrate Population Health and VBC Technology in CM Modernization and UM Modernization efforts (where possible), while ensuring business continuity, positive provider engagement and the ability to deliver 2022 affordability goals and LOB commitments.
