Description A Mental health Clinic in the St. Paul area is seeking an Outreach Coordinator to join their team. This person will be making outbound calls to UCare members to offer them a voluntary service, document calls, and schedule appointments for the program staff to meet with medical professionals1.Make out-going initial and follow-up calls from a monthly roster spreadsheet provided by SNBC Team Leader.Introduce health plan service to members reached by phone, offer an appointment and navigate questions from members. Be ready to provide UCare Navigator contact for members who have questions.

Schedule appointment on SharePoint calendar, email Care Coordinator with scheduled appointment and document on spreadsheet.

Document all call outcomes, including unable to reach, voicemails, declined appointments and scheduled appointments on spreadsheet.

Communicate with team leaders, care coordinators and program assistant any pertinent information regarding next steps and appointments.

Meet weekly and as needed with UCare SNBC Team leader for supervision and consultation.

Provide excellent customer service and detailed documentation.

Requirements

Demonstrated flexibility to adapt to changes in procedures and job assignments

1 year of business/office experience at minimum

BA/BS degree in business, communications, or related field preferred; high school diploma or its equivalent required

Knowledge of database and file management

Excellent attention to detail

Past experience with Microsoft Project, JIRA, Mavenlink, and NetSuite

Comprehensive knowledge of Community Outreach

Expertise in Outreach Events

Past experience with the Microsoft Office Suite - Word, Excel, PowerPoint, and Outlook

Ability to multitask efficiently and prioritize work
