Become a part of our Care Management Network in the Olive Branch/Southaven, MS areas.
We are seeking Care Managers interested in joining an interdisciplinary team in providing individualized health coaching and motivational support to prevent unnecessary hospitalizations among our members. Our ultimate goal is to improve the health and well-being of these we serve.
Our nationwide programs support individuals who have chronic conditions and functional limitations. All positions start as part-time with the opportunity to work into full time. All visits are performed in members home. We also offer telephonic intervention.
Compensation will be $75.00 for the initial assessment and $45.00 for all visits thereafter. Telephonic intervention calls compensate $20.
Requirements:
Current nursing license with no flags
RN- with 1 year geriatric or home health experience
LPN required minimum 3 years geriatric or home health experience
Strong computer skills including web navigation, applications/software programs as well as Microsoft Outlook.
Valid drivers license, car insurance, dependable automobile for home visits to member.
Primary duties will be to:
Educate members on disease management
Increase members independence
Advise members on prescribed diet
Medication reconciliation
Identify any gaps or barriers in members care
Long-term goals are to reduce re-hospitalization and ER visits and improve quality of care and outcomes. No direct care is provided, all visits are hands off nursing.
