Part-Time, 0.75 FTE, 30 Hrs/ weekPosition Summary:The Resident Care Coordinator is responsible for the coordination, development, implementation, and evaluation of resident care and clinical practice to ensure a high standard of care is delivered. As a clinical resource and leader for the care team, provides guidance, supervision, and intervention in the delivery of resident care and the care planning process in accordance with all regulations and policies.Minimum Qualifications:Montana Licensure (LPN) or Montana Licensure (Registered Nurse)
American Heart Association BLS
Preferred: 6 months of experience in similar role
Essential Job Functions:Coordinates, supervises, and evaluates the care of residents in a skilled nursing, assisted living, or similar facility.
Schedules and trains care staff, and ensures that clinical services are delivered in accordance with regulations and professional standards.
Communicates with families and medical professionals as needed, and ensures care plans for residents are properly documented and updated.
Provide care and compassion to foster the resident’s physical and emotional well-being.
Participant in performance improvement activities (such as Fall Risk team or Infection Control Committee) to improve the safety and care of the Residents.
Knowledge, Skills and AbilitiesDemonstrates sound judgement, patience, and maintains a professional demeanor at all times
Ability to work in a busy and stressful environment
Strong interpersonal, verbal and written communication skills
Ability to work varied shifts
Self-directed, completes assignments accurately, thoroughly and with minimal oversight
Detail oriented, organizational skills and the ability to prioritize
Strong emotional intelligence, interpersonal and teamwork skills
