Case Manager/Care Advisor - Mid North Coast - Immediate Start- Full Time Position
- Opportunity to partner with our Home Care Package Clients and dynamic team

- Based in Coffs Harbour with occasional interstate travel.
Working autonomously, you will use your skills and experience to oversee the case management of our Home and Community Aged Care Clients in the Coffs Harbour region.
What will your role look like?You will use your extensive skills and experience to oversee the Case Management of Aged Care clients, ensuring that they have a flexible, responsive support service/intervention tailored to their individual needs and interests.- To facilitate a collaborative service approach to identify supportive solutions for people
- Assess, coordinate and review client care needs within their funding allocation.
- Coordinate a service response that is directed to ensuring housing is maintained for the older person and that their care needs are met so they can continue to live in the community
- Delivery of creative strategies and solutions that deliver exceptional client outcomes.
- You will be responsible for ensuring the delivery of flexible and responsive services tailored to an individual's needs and interests, whilst also providing advocacy and referral services to clients.
- Collaborate with both government and nongovernmental support agencies within the community care sector in order to achieve improved housing, health and well-being outcomes for clients.
- Have a genuine passion for care, balanced with a desire to maximise services and offer commercially sustainable case management in line with the organisation's strategy.

- Strong commitment to growth
- Demonstrated experience in case managing individual clients is essential to be considered for this position.
- High level of communication skills and a "no judgement" approach
- Understanding of the complexities associated with situations of ageing
- Mental Health experiencehighly desirable
- Relevant qualifications in Allied Health, Nursing, Social Science or related field is desirable.
- Highly developed planning, sales and negotiation skills
- Demonstrated experience in managing the contractual requirements of government programs to ensure compliance, regulatory standards, and organisational policy and procedure
- High degree of computer competency with an ability to learn new software programs including Orca & Procura
- Registered and insured motor vehicle and a current unrestricted Australian drivers' licence
- Influenza and COVID 19 Vaccinations
