The pediatric staff pharmacist works in the decentralized pharmacy satellite and provides collaborative pharmaceutical care to the Children s Hospital of New Jersey including NICU and PICU, and the emergency department.QUALIFICATIONS:Required:Qualified candidates must have a Bachelor of Science or Doctor of Pharmacy degree from an ACPE-accredited school of pharmacy
NJ license as a Registered Pharmacist
Hospital pharmacy experience in pediatrics
Candidates should also be self-motivated, have good communication, organizational, and interpersonal skills
Preferred:Completion of a PGY1 Pharmacy Residency in general practice.
The major job responsibilities of this position are: pharmaceutical care to pediatric patients in the Children s Hospital of New Jersey and the pediatric emergency department, be a resource to the physicians, residents, nursing, and other healthcare disciplines on the unit, and provide drug distribution services including sterile compounding.The following activities are components of the position to assist in meeting the major job responsibilities:Provide comprehensive pharmaceutical and critical care services to neonatal, pediatric, adolescent populations in the CHONJ
Participate in daily patient care activities and discuss medication order clarifications with the prescriber, document any changes in patient and pharmacy records, and inform others of medication order changes.
Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions and provide pharmacokinetic consultations and other dosing as required.
Prevent, detect, monitor, document, and report adverse drug reactions and medications errors.
Participate in the drug therapy management of medical emergencies as needed.
