We are seeking compassionate Care Managers in Roswell, Rio Rancho, New Mexico and surrounding cities to join our Amazing Care Management Team by changing the way In-Home Health Care is delivered.
Description: As a Care Manager, you will engage clients in a collaborative relationship which empowers the member to manage their physical, environmental and psycho-social health issues, to improve and maintain lifelong well-being while allowing them to remain at home.
Responsibilities: Provide "best-in-class" complex care management to vulnerable clients, the frail elderly, the chronically ill and the functionally challenged. The Care Manager position includes:
Conduct assessments and on-going visits to address chronic medical conditions and behavioral challenges
Provide resources and support to clients
Work closely with the clients multidisciplinary team (providers, case managers and other health care professionals)
Thorough, timely documentation and training using web-based portal
Effectively communicate specific program goals and outcomes
Participates in quality improvement activities
Experience Required: Care/Case Management in a Community-Based or In-Home Setting:
ï Licensed Registered Nurse - one year
ï Licensed Practical Nurse - three years
ï Licensed Master Social Worker or Certified Care/Case Manager - one year
Must have own computer with high speed internet connectivity (DSL or cable modem) and be proficient with Microsoft applications: Outlook, Word and Excel
Must pass background check, drug screen, current TB, CPR/First Aid, valid driver's license, car insurance, and access to an automobile for home visits to member is required
Good communication skills, and ability to work with a multi-disciplinary team
Compensation: Per in-home care management documented visit
