Mennonite Village in Albany Oregon is a not-for-profit CCRC facility that provides 24 hours/7 days a week care of varying levels. Lydia’s House is our Memory Loss Unit designed to care for elderly persons who have Alzheimer’s disease or related dementia. Due to the size of these units and the unique level of care; all staff may be required to provide some services primarily performed by others. We are seeking Med Aide/Team Leader to work variable 12 hour shifts. Must be able to work weekends and holidays. Shift differential for hours worked between 10pm & 6am. If you are not a certified CNA in the state of Oregon, you must be able to complete Company paid CNA certification within one year of hire. ESSENTIAL FUNCTIONS:

Distributes medication to residents consistent with accepted nursing standards.

Orders medications as needed.

Answers phone and directs phone calls.

Cleans and restocks medication cart routinely; orders new medications as needed; removes outdated and discontinued medication; non-medication items are not to be stored on cart.

Completes documentation responsibilities accurately, including medication sheets, treatment sheets, progress notes, physician’s orders, occurrence reports, lab results, x-ray results, data for service plan, and all other related forms.

Practices standard precautions with every resident. Practices additional precautions per direction of nurse on duty.

Acts as point of contact for physician concerning resident condition changes, order changes, diagnostics, or transfer orders. Contacts other facility staff and family members as needed.

Coordinates services to meet residents’ secondary needs such as dental services or the repair of hearing aides and glasses.

Assist residents with: activities of daily living (ADLs), bed making, diet and hydration, bathing, and personal laundry as time allows.

Monitor health status of residents and report changes to Administrator or nurse.

Assist with dietary tasks, such as meal and snack preparation, serving, set-up, clean up, and dish washing. Eat meals with residents as designated.

Perform housekeeping tasks such as laundry, vacuuming, mopping, organizing, cleaning refrigerator, etc.

Complete documentation as required, such as resident meal intake, vital signs, etc.

Maintain confidentiality of resident information at all times.

Assist in planning, implementing and evaluating therapeutic programming. Plan and lead activities such as exercise, trips, crafts, parties, musical events, etc.

Attend in-services, and unit meetings. Attend out of facility educational sessions as directed.

Attend patient care conferences as required.

Design and update bulletin boards, message boards, menu boards and appropriate monthly decorations.

Provide direction to volunteers working on the unit.

Monitor supplies and keep manager aware of supply needs.

Follow all Lydia’s House and Mennonite Village Policies.

Know how to operate all equipment in Lydia’s House and report any problems.

Regular attendance and punctuality is an essential function of the job.

Abides by the HIPAA policies and reports to a supervisor, breaches (or violations) of our HIPAA policies.

Immediately reports identified hazards or hazardous conditions to a supervisor.

Immediately reports adverse events and near misses to a supervisor.

Attends mandatory training on HIPPA and compliance annually.

QUALIFICATIONS:

Supportive of Mennonite Village Philosophy.

Ability to describe the Social Model of care after appropriate teaching, and be willing to execute this model in Lydia’s House.

Ability to be flexible, patient, creative, willing to learn and be motivated to enhance the lives of residents residing in Lydia’s House.

Ability to focus on each resident as an individual.

Ability to work as a team.

Ability to professionally handle emotional and behavioral changes in Lydia’s House residents and families.

Basic understanding of the principles of pharmacology. Understands or is capable of learning how drugs interact with the human body (pharmacodynamics).

Ability to work with and communicate effectively with the elderly.

Capable of lifting 10-100 lbs., with occasional lifting of residents weighing over 100 lbs.

Ability to safely move frequently during workday, including carrying, lifting, standing, sitting, bending reaching, and kneeling.

Ability to push and/or pull medication cart (50 lbs.) and other equipment.

Visual and auditory acuity within normal limits

Must be flexible and responsive to change on short notice.

Sufficient English and language skills to follow instruction from staff. Must be able to understand and respond properly to residents’ needs.

Ability to accurately document activities in writing.

Self-motivated.

Ability to work flexible hours, weekends, and participate in holiday activities
