The LPN functions performs tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care. Works as part of a care team to improve the completion of quality requirements, increase throughput, enhance efficiency, and enrich the overall patient experience. The Licensed Practical Nurse (LPN) provides direct and indirect nursing care individualized to the patient’s needs. Collects and records relevant patient health data accurately Evaluates health data to identify patient problems Implements and documents specified interventions Identifies deviations from expected patient outcomes Adheres to approved, research-based practice standards The Licensed Practical Nurse (LPN) performs and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedures The Licensed Practical Nurse (LPN) maintains and documents testing reagents/supplies and equipment according to established laboratory procedures Advocates for patients to ensure that care preserves and protects the patient’s autonomy, dignity and rights. Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity Assures that patients have informed consent signed for indicated procedures The Licensed Practical Nurse (LPN) maintains current licensure, certification, and competencies. Provides proof of current BLS validation to supervisor. Completes required unit credentialing according to departmental policy The Licensed Practical Nurse (LPN) provides to supervisor evidence of completion all required annual training and competency validation Practices within the New York State-defined LPN scope of practice 100% of the time Acquires and maintains current knowledge and skills in his/her area of practice. The Licensed Practical Nurse (LPN) performs other duties as requested and observed by supervisor or manager. Qualifications Education: Graduate of a Practical Nursing program required Experience: 1 year of healthcare related field experience preferred License/Certifications: Current NYS LPN License and registration as a Practical Nurse or a valid interim permit BLS through an accepted agency must be obtained within 6 months of hire Additional credentials as determined by specific work unit required Skills: Clinical Skills Multi-Tasking Interacting with People Analysis & Evaluation Take Initiative Physical Requirements: Heavy Work - Exerting up to 100 lbs. of force occasionally, and/or up to 50 lbs of force frequently, and/or up to 20 lbs, of force constantly to move objects. Occasional climbing, crouching, grasping, kneeling, lifting, pulling, pushing, reaching, stooping, visual acuity (color, depth, perception). Frequent balancing, typing/clerical/dexterity, hearing, repetitive motion, standing, talking, walking, seeing/monitor/computer use. Frequent fit testing required. Hazards: Occasional burns, chemical, electrical, mechanical, radiation/radiant energy. Frequent bodily fluids/bloodborne exposure. Working Conditions: Occasional noise, dust, fumes, gases, mists, odors. Travel: Occasional travel - the job may require travel from time-to-time, but not on a regular basis. Pay Range Minimum USD $21.11/Hr. Pay Range Maximum USD $31.67/Hr.
