Job Summary RN Hospice liaison needs a strong working knowledge of hospice rules and regulations, strong listening skills, patience, compassion, high energy, and have excellent time management, interpersonal, relationship building, networking and communication skills. The RN Hospice Liaison will serve as a contact to connect Sanford Fargo Hospice with the medical centers and community. The RN Hospice Liaison will serve as an advocate for hospice patients admitted to the hospital, coordinate hospice admissions, assist with referrals to Sanford Fargo Hospice, educate hospital patients and families about hospice care, assist with end of life education for staff, complete hospice consults, facilitate communication between Sanford Hospice and health care providers, hospice care coordination, develop positive working relationships with health care team, and participate in community events. The nursing process includes the documentation of human responses to actual or potential health problems in the home health environment. This practice includes dependent, interdependent and independent functions. Collaborates patient/resident and family, other inter-professional colleagues, such as physicians, to plan, implement and evaluate care. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned setting. Cares for patients/residents in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up for patients/residents in the hospice setting. Responsible to implement the nursing process, the coordination and continuum of care, patient/resident assessment, patient/resident education, triage, and various other nursing interventions. Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct. Bachelor's degree preferred. At least 1 year of work experience as nurse in hospice or home health care required.

Qualifications Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelor's degree in nursing preferred. Minimum of one year of professional nursing experience preferred. Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications. Depending on location, a valid drivers license may be required. Basic Life Support (BLS) required. Certificate for Hospice and Palliative Nurse (CHPN) preferred.
