Status: Full-Time non-exempt. Must be flexible based on client schedules. Work schedule to be discussed in the interview.Duties include: Work directly with clients that are transitioning from correctional facilities/incarcerated settings back to the community. Duties include assessments, case planning, meeting with clients for individual support and group facilitation. Serve as positive, pro-social role model to assist clients in establishing employment and accessing other needed resources. Maintain on-going communication with referral sources, crisis response, records and reports. Attend weekly case management meetings and oversee accountability.Requirements: This position requires a minimum of a Bachelor's Degree in a Social or Behavioral Science program from an accredited college or university. Work experience may be substituted for academic studies exchanging one year of work experience in a related field for one year of academic education. Total work experience needed in lieu of the combination of education and work experience is five years. The position requires the ability to work well with offenders in a rehabilitative program, conduct formal assessments, develop treatment plans and monitor client's progress towards their goals. The position requires networking with other service providers and referral agencies and computer knowledge. This position also requires the ability be a team player, possess strong oral and written communication skills, and have the ability to understand and work within the agency mission and various regulatory guidelines.This position requires the ability to work well with clients, service providers, as a team and follow the agency mission and contract requirements.
