JOB SUMMARY

The RN Nurse Navigator is designed to enhance initial care communications through the building and preserving of on-going patient relationships and community relationships that refer patient care in home health and hospice to Excell. The Nurse Navigator will deliver clinical information to referral sources and electing patients/family members in the initial format when client is in the election process or referral process. This position will assist both traditional business development and clinical nursing and therapy roles to help the initial patient/family experience begin a quality transitions of care and to promote the brand recognition of quality within the community.

DUTIES & RESPONSIBILITIES

1. Make visits to new referrals in home health and hospice.

2. Provide patient/family discharge planning education as it relates to transitions of care in home health and hospice and any other general clinical education related to their diagnoses.

3. Prior to patient visits, review patient H&P, orders, relevant notes, and coordinate with intake, clinical supervisor, director of clinical services, and/or executive director for any additional relevant information, if necessary.

4. Make contact with patient/family and provide on-site warm hand-off at place of referral (e.g. hospital, SNF, facility office, etc.).

5. When referral hand-offs are not available, report to office for market review/analytic time to review market opportunity and/or connect via email/phone with community about your role in transitions of care and Excell.

6. If warm hand-offs are back-to-back, day-to-day, daily accountability is required to clinical director and/or executive director about daily routine and work load.

7. Obtain and work hospital hold report daily. Coordinate with office care coordinator.

8. When these duties are not consistent, or there displays gaps in volume to these activities, this role also includes traditional marketing to an assigned area:

  • Attend weekly marketing team meeting with executive director.
  • Study assigned market area and manage routine calls to assigned area.

Qualifications

1. Must have graduated from an approved school of nursing and be licensed in the state where practicing.

2. Minimum of two years of experience, at least one of which is in the area of public health or home care nursing is preferred.

3. Maintains a current CPR certification.

4. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

5. Self-directed and able to work with minimal supervision.

6. Demonstrates excellent observation, problem solving, verbal and written communication skills; nursing skills per competency checklist.

7. Shows ability to organize and prioritize workload independently.
