That means you'll have an opportunity to make an impact on a huge scale - as part of an incredible team culture that's defining the future of behavioral health care.Service Coordinators work with individuals diagnosed with serious and persistent mental illness (SPMI) or individuals with a qualifying mental illness who apply for or are receiving certain behavioral health (BH) program(s) and services. The goal of the Service Coordinator is to ensure that each Recipient has the best chance of enjoying a healthy and productive life by deploying a person centered approach to identifying treatment needs consistent with their desire, needs and values and providing fully integrated services that effectively support individuals in achieving stability and independence.This position is a combination of field-based and telephonic work. We have many territories located along the I-5 corridor as well as the greater Burns, La Grande, Bend and Medford areas. Candidates must be willing to be in the field approximately 50% of the time. This will be a Telecommute / Work From Home position when not in the field.Primary Responsibilities:

Conduct face to face needs assessments/ reassessments using pre-defined tools and examine relevant history, including findings from the evaluation of eligibility, medical records, an objective evaluation of functional ability, and any other records or information needed to develop a person-centered service plan (PCSP)

Conduct medical appropriateness reviews based upon the Recipient’s need for the services as identified in the functional needs assessment and documented in the annual PCSP

Perform transition management and planning for individuals transitioning among service settings

Assist Recipients and their families/caregivers with connections to appropriate psychiatric, medical, and psychosocial referrals and services

Identify and remove barriers to procurement, delivery, participation in and success of services

Provide supportive follow-up, monitoring and education as indicated, in-person and telephonically

Coordinate the implementation of the PCSP for Recipients who do not have support or assistance from a community organization

Partner with designated external providers, programs, entities to address the needs, gaps in care, and recovery goals of complex and high risk Recipients

Continually monitor the health status of Recipients and engage those who are struggling, not meting PCSP goals, or having difficulty stabilizing, through face to face and telephonic support

Establish cooperative relationships with Oregon State Hospital sites, Transition Age Youth Residential Home and Secured Residential Treatment Facility staff and the Provider community in order to improve access to services and ensure that Recipients are served within proper standards of care

Credentialed for the requirements for a Qualified Mental Health Professional (QMHP) in the state of Oregon (Applicants must have documentation from the state of their approval as a QMHP) OR be Independently licensed by a State of Oregon Licensing Board.

2+ years’ experience post degree with Behavioral Health levels of care and settings

Computer / typing proficiency to enter / retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications

Dedicated workspace with access to high speed internet from home

Ability to accommodate daily field-based travel within your designated territory up to 50% of the time

Preferred Qualifications:

Experience in clinical case management conducting face to face assessments, examining individual’s relevant history and developing person-centered service plans

Experience working in an environment that required coordination of benefits and identification of community-based resources

Experience in clinical case management telephonic outreach and engagement

Experience with Title 1915(i) Home and Community-Based Services (HCBS)

Experience working with Transition Age Youth (TAY) (ages 17.5 to 26)

Previous experience in a managed care environment
