Job DescriptionAre you ready to expand your nursing skills, while aiding in the reduction of hospital readmissions and helping patients get back to their daily life faster? How about dipping your toes into population health nursing and impacting the lives of patients and their families, with no limitation on how long it may take to help that member meet their desired health goals?We are looking for full-time RNs to join our team, in the role of Telephonic Case Manager!Required hours are 8:30am - 5:00pm with occasional late coverage and weekend coverage. This role is work from home.The Nurse Case Manager is responsible for telephonically assessing, planning, implementing, and coordinating all case management activities with members and guides the member telephonically in defining their health goals and taking appropriate steps to achieve their optimal state of wellness. The Nurse Case Manager educates on many topics including: disease process, treatment understanding, and medication role. Through proactive development of a course of action, they address issues identified to enhance the short and long-term outcomes. This Nurse Case Management role utilizes a collaborative process of assessment, planning, facilitation, and advocacy for options and services to meet an individuals benefit plan and/or health care needs.Required QualificationsRN with current unrestricted state licensure required- 3+ years of med/surg experience- Willingness to obtain additional state licenses

Preferred QualificationsCompact RN license- Case Management in an integrated model- CCM- Experience with all types of Microsoft Office including Powerpoint, Excel, and Word

EducationAssociates in nursing required- Bachelor's degree in Nursing preferred.
