Job Description
As the
Community Program Specialist you will provide in-house or community based support and supervision to individuals with disabilities/barriers to employment by ensuring those individuals' needs are met, goals are implemented, and a safe environment is maintained.Duties will also include but are not limited to:- Assist participants on caseload in identifying needs and writing goals that are relevant to their individual program plans.
- Collect in depth information about a participant's situation and functioning to identify individual needs in order to develop a comprehensive program plan according to partner input an contract specifications.
- Communicate with other interdisciplinary team members regarding participant's progress toward established service plan goals.
- Conduct ongoing meetings with participants to ensure quality service delivery and conduct progress reviews of goals in accordance with program/contract guidelines.
Qualifications
- Associate’s degree (Social Work, Psychology, Human Services, or related field) and 4 years’ of experience successfully working with individuals with disabilities/barriers to employment required. OR
- Bachelor’s degree(Social Work, Psychology, Human Services, or related field) and 2 years’ of experience successfully working with individuals with disabilities/barriers to employment required.
