Hospice Travel Assistant Director of Nursing, South CarolinaDescriptionPOSITION SUMMARY:The Assistant Directorof Health Services (ADHS) coordinates the provisions of Hospice services foreach patient and family, coordinates clinical services for the Hospice agency,supervises the interdisciplinary team members, and provides outreach to thecommunity.KEY RESPONSIBILITIES:1. Primarily responsiblefor taking and coordinating referrals and admission through the establishedorganizations policies and guidelines.2. CoordinatesHospice services for each patient and family.3. Responsible forcoordinating and training of personnel to nursing policies and procedures.4. Coordinatesadmission processes through assignments to clinical staff and/or performingfield admission visits.5. Coordinatesadmission paperwork to ensure timely completion and accuracy.6. Makes fieldvisits to evaluate appropriateness, determine eligibility for services andpresent Hospice program.7. Assigns clinicalstaff to patients and families based on the patient needs, including Chaplain,Social Workers, Volunteers, Dietary Counselors, therapists for PT/ST/OT, andbereavement coordinators.8. Implementationof employee relations practices necessary to establish a positive partner –organization relationship and promote a high level of partner morale.9. Follows allcompany policies and procedures, state and federal laws and regulations andcharged with reporting violations to the appropriate supervisor.10. Ability torespond to change productively and handle additional projects as assigned.11. Communicatesassessment findings to Attending Physician and initiates treatment plan perphysician’s orders.12. Orders andapproves DME and nursing supplies.13. Assists inprovision of training on Hospice issues to clinical staff, patients andfamilies, healthcare centers, nursing facilities, acute care facilities andcommunity groups.14. Performsroutine chart audits to ensure timeliness and accuracy of documentation and MDorders.15. Identifies,completes and follows up on quality assurance performance improvement reportsincluding monitoring of infections and Opportunity Occurrence Program Reports.16. Facilitatesteam meetings and care plan review.17. Scheduleson-call RN duties and provides backup coverage and administrative on-callduties.18. Communicateswith Attending Physicians and Medical Director on routine basis as indicated.19. Completescompetency assessments for nursing, certified nursing assistant, and aidepartners.KNOWLEDGE, SKILLS,ABILITIES:1. Commitment tothe Hospice concept and standards.2. Ability tocommunicate effectively verbally and in writing with individuals and groups.3. Ability tohandle confidential matters.4. Ability to worka flexible schedule.5. Ability to workeffectively, independently and within an interdisciplinary team.6. Participate incenter/agency surveys (Licensure/JCAHO) and any subsequently required reports.7. Attend andparticipate in continuing educational programs to keep abreast of changes inyour field as well as to maintain current license/certification as required.8. Attend andparticipate in mandatory inservices.9. Honor patients/residents’rights to fair and equitable treatment, self-determination, individuality,privacy, property and civil rights, including the right to wage complaints.10. Comply withcorporate compliance program.11. Reportjob-related functions/tasks that involve occupational hazards includingexposure to blood and body fluids and others as necessary.12. Follow establishedsafety regulations, to include fire protection and prevention, smokingregulations, infection control, etc.13. Follow establishedsafety procedures when performing tasks and/or working with equipment.14. Perform otherrelated duties as necessary and as directed by supervisor.#CBQualificationsPREFERRED SKILLS ANDEXPERIENCE:1. BSN degree preferred.2. Minimum of fiveyears nursing experience preferred.3. Three to fiveyears management experience preferred.4. Experience in Hospiceor HomeCare preferred.LICENSURE,CERTIFICATION, AND EDUCATION REQUIREMENTS:1. Current RegisteredNursing License in the state where care is provided.2.
