Job ResponsibilitiesCareers at DHEC: Work that makes a difference!Pursuing Excellence, Inspiring Innovation, Promoting Teamwork, and Embracing ServiceUnder supervision of CYSHCN Program Coordinator, provides culturally competent nursing care for children and youth with special health care needs. Provides nursing care per agency policies/procedures, standing medical orders, patient specific medical orders. Promotes quality customer service and maintains strict confidentiality. Implements and adheres to agency and program policies, procedures and expectations. Participates in disaster preparedness per DHEC Lowcountry Plan during hazardous weather or declaration of state of emergency.Population Focused Care: Develop community relationships with providers, clinics, and school systems promoting case management of at-risk populations. Assess, Plan, Implement (Care Coordinate), Monitor, and Evaluate services required to meet client's healthcare needs. Faciliate care coordination, communication, and collaboration between clients, providers, and community resources to achieve goals and optimize (maximize) outcomes based upon individual assessments and client needs. Collaborate with community partners facilitating case management of Children and Youth with Special Healthcare Needs through advocacy, communication, education, identification of service resources and facilitation of access to these services. Identifies family needs and prioritizes for children seeking CYSHCN services formulating a plan of care. Determine eligibility for financial assistance per DHEC policy. Authorize services for eligible persons as needed. Serves as care coordinator for assigned caseload and implements plan of care through visit with families. Functions as member of an interdisciplinary team. Assists with screening and physical assessment of campers on check in days at Camp Burnt Gin, as assigned. Document care/services when provided per agency policies/procedures.Continuous Quality Improvement (CQI): Participates in development and review of CYSHCN policies and procedures related to delivery of CYSHCN services. Collects, analyzes and reports on characteristics of assigned caseload, services provided and outcomes. Participates in periodic review of caseloads during team review of persons being served.Management: Provides clinical consultation to CYSHCN team members.Leadership: Represents office or region on committees, workgroups,and community events as assigned.Maintains clinical competency as required by Office of Public Health Nursing relevant to practice and disaster preparedness.Minimum and Additional RequirementsState Minimum Requirements: Licensure as a registered nurse by the South Carolina Board of Nursing. Some positions require a valid driver's license. Also, some positions require certification in Basic Life Support and/or Basic/Advanced Cardiac Life Support. Graduation from an accredited school of nursing.Institutions of Higher Learning must be recognized by the Council for Higher Education Accreditation.Agency Additional Requirements: Bachelor of Science in Nursing and current licensure as a Registered Nurse by the State Board of Nursing for South Carolina or a compact state. Must have a valid driver's license, satisfactory driving record, and use of a personal vehicle.Knowledge and skill in the techniques and practices of the nursing profession. Knowledge and skill in public health nursing theory and practice including social and public health sciences. Ability to plan, implement, and evaluate care. Ability to prepare and provide health teaching and counseling. Ability to apply Continuous Quality Improvement (CQI) concepts to practice. Ability to apply judgment while adhering to standing orders, policies/procedures and laws governing Nursing. Ability to interact in a culturally competent, professional manner. Knowledge of community resources. Ability to utilize computers/related technology efficiently.Must have current driver's license or ability to obtain transportation. May be required to use personal vehicle. CPR/First Aid certification within first 6 months and then as required. Must be able to lift 30 pounds and stand for long periods. Travel for meetings and training. May have to travel overnight for training.
