Previous experience in home care is not necessary, but common judgment and a willingness to learn and help others is required.

Description

The referral coordinator is key personnel to the daily operations of our home care office. The Referral Coordinator will work in a fast pace environment with heavy phone interaction with caregivers and clients. The Referral Coordinator will utilize company referral platform to ensure information is updated, accurate, and compliant.

Essential Functions

-Assist client base with referral of caregivers based on their requested services needs

-Develop strong relationships with caregivers and clients to ensure outstanding client satisfaction.

-Participate in the caregiver recruitment and screening process.

-Conduct telephone intakes with new and/or potential clients and track all prospects.

-Participate in internal audits of files to meet all company, state, and federal quality and compliance standards.

-Complete follow-up marketing home visits with existing clients as needed.

-Required to be on-call on a rotating basis.

-Perform initial home visit with potential clients to explain referral services, and obtain signed intake document as needed.

-Perform other duties as assigned.

-Excellent customer service and interpersonal skills

-Ability to multi-task, coordinate, and plan for daily, weekly, and on-going referrals

-Strong organizational skills and detail oriented; good documentation skills

-Intermediate level of computer skills to include knowledge of Microsoft Office

-Reliable transportation needed for home visits as needed
